Mood ChartsTo use these charts you simply download them in Word document, print the chart you want (you may want to save it to your hard drive first) and then fill in the Name and Date at the top. Fill in each symptom that you wish to track and at the end of the day, mark the highest level of that symptom that you have seen on a scale of 0 to +5. Some possible symptoms would be mania, depression, aggression, anxiety or lethargy. Be sure to note events on the chart that could be relevant, such as disturbed sleep, missed medication, or unusual or relevant comments the child has made. For example, if depression is a symptom you are tracking, marking +5 would indicate that the person was totally despondent and likely suicidal, a +2 would mean the person was down but somewhat functional, and a 0 would mean no depression. These charts can be filled out weekly and used to keep a long term record to help make medication decisions and other planning. This second chart is similar to the first, but has a rating scale of -5 to +5 which is handy for charting such symptoms as mania (+5) vs. depression (-5) or high energy (+5) vs. lethargy (-5). Use a different color to record information on each symptom, and then you can use the graph at the bottom to chart all of the symptoms at once, and see patterns in symptoms and medication changes.
