Somatic Complaints

Somatic means physical. Somatic complaints are aches, pains and general physical distress. Depression can often cause such complaints. Kids who never have headaches suddenly begin complaining of frequent headaches or teens suddenly start complaining of a lot of nausea or stomach distress. These are the two most common somatic complaints. Fatigue, changes in appetite, carbohydrate cravings, weight gain or loss, muscle tension, muscle or joint aches and pains, and sleep disturbances can all be physical symptoms of depression as well. Some doctors feel that a significant number of children are more likely to show such physical signs of depression as the primary noticeable symptoms than adults typically would. It should also be noted that anxiety can be an issue for some children with bipolar disorder and that anxiety can also create physical complaints such as headaches, stomach aches. If a child or teen is having a panic attack, physical symptoms may include: racing heart/palpitations, dizziness, feeling that they are going to pass out, feeling that they are going to die, feeling very cold especially in the extremities, and other symptoms as well.

A child with somatic complaints is not making these complaints up for attention-seeking purposes. The pain or discomfort your child is experiencing is real, even if it stems from the same brain processes that your child’s mood disturbance stems from. Do be aware that children can also develop other medical problems while being treated for bipolar disorder and it may be necessary to have a doctor check your child for possible physical/medical reasons for their discomfort. So if your child exhibits new or increasing physical distress of any sort listed above, please discuss this with her/his tdoc, pdoc, and pediatrician. (Even if a medical explanation is found, this new diagnosis and the related discomfort will likely affect your child’s or teen’s mood, so either way the mental health treatment team needs to know.)
